Nomination Form

Every nomination must include all of the information below. Please include as much information as possible which may also include written articles or letters of support from coaches. If more information is needed, a director will contact the sponsor & help produce a more complete resume for the nominee.

Please click HERE for eligibility requirements. The deadline for any nomination is October 31^st of each year. Any nomination received after that date will be held and considered for the following year.

If a nominee is selected for induction into the Hall of Fame, he/she will receive a letter in the mail during the month of December with complete details and directions. This letter will include all dates, any information you will need, and contact numbers. We will also be asking the nominee to send us two pictures: a portrait-style picture and an action shot or two.

Anyone needing assistance with this form is encouraged to contact one of the directors listed on the website. The sponsor will then be contacted by a board member to resolve any questions or issues.
